It is important that you sign the form, record your drivers license information, and bring your license to your appointment.
Your physical will focus on your eyesight, heart, and the physical and mental ability needed for you to safely perform the demands of your job. Please bring your glasses.
COST
DOT Physicals are $140.00 and insurances presently do not cover the service.
3 month follow up DOT exam for some conditions $50.00.
Payment is expected at time of service.
